Damage Assessment Instructions

IMPORTANT: Complete this form to help Maui County qualify for state and federal disaster assistance.

The information you provide here is part of the Preliminary Damage Assessment (PDA) process — a joint effort between county, state, and federal officials to determine the impact and magnitude of damage caused by the disaster.

The PDA is the required starting point for the Governor to request a Presidential Major Disaster Declaration under the Robert T. Stafford Disaster Relief and Emergency Assistance Act (Stafford Act). The data collected through this self-reporting helps FEMA validate damage, assess unmet needs of individuals, businesses, and communities, and determine eligibility for Individual Assistance (IA) — which provides grants for temporary housing, home repairs, and replacement of damaged belongings — and Public Assistance (PA) — which provides grants to county and state governments for infrastructure repair.

Please be as thorough and detailed as possible. The more accurate and complete your report, the better it supports Maui County's request for federal aid. Take lots of photos with geotags if possible and upload them with your report.

What to include in your description:

  • Type and location of all damage — be specific about what is damaged and where
  • Type and estimated volume of debris — include piles, fallen trees, building materials, etc.
  • Hazardous materials or waste — note the type and approximate amount
  • Damaged batteries — especially damaged Lithium-ion batteries, which must be kept separate from all other batteries and stored outdoors in a non-flammable container away from structures, if safe to do so

If your property has multiple dwelling units (such as an Accessory Dwelling Unit / ADU — also known in Hawaiʻi as an ʻOhana Dwelling, Supplemental Dwelling Unit / SDU, or Additional Dwelling Unit), please clearly indicate which unit you are reporting by including:

  • The specific address or unit number for each unit
  • A description identifying the unit (e.g., "main house," "ADU/ʻohana unit above garage," "detached SDU," "garage conversion," etc.)
  • If reporting multiple units, submit a separate form for each unit and clearly distinguish them by address and description

CHOOSE THE CORRECT REPORT TYPE BELOW BASED ON YOUR DAMAGE:

DEBRIS CLEARANCE: Use this for debris management only. This includes debris from your own property OR debris that originated outside your property line but ended up on your property. This type does NOT include damage to your primary dwelling or any additional units (ADU, ohana, SDU, etc.).

RESIDENTIAL DAMAGE: Use this for damage to your primary dwelling and property. If you have multiple units on your property (such as an ADU, ohana dwelling, garage conversion, or SDU), you must submit a separate form for EACH unit. Clearly indicate which unit you are reporting by including the unit number, address, and a brief description (e.g., "main house," "back ohana unit," "detached SDU").

COMMERCIAL / INDUSTRIAL DAMAGE: Use this for all businesses, including farms and agricultural properties.

PUBLIC FACILITY DAMAGE: Use this for all public infrastructure, resources, structures, facilities, and rights-of-way.
